How do I get the PUK for my SIM card?

You will need to provide the following details for verification purposes before you can be given the PUK for your SIM card:                                

-Name and DOB

-The phone number to be unlocked                        

-Year of purchase (YOP)                                          

-Four frequently dialed numbers (4FDN)                          

Once you have the above info together, please call us on 123 and follow the prompts to get your PUK code.
